what happens if you get 18inch rims and the tyres bust or the wheel f's up?
the spare tyre is for 16's much smaller so what do you guys usually do?
can the spare tyre room be fixed to fit spare 18's?
When you get bigger rims - say 18" - you will usually get a lower profile tyre to retain the same rolling diameter - otherwise you need to re-calibrate the speedo.
If you retain the same rolling diameter you can also use the same 16" spare tyre no problems.

same rolling diameter is very important for the spare tyre, if your put two different type diameter tyres on the front ( FWD car ) then your differential will constantly be rotating on straight line drives, causing premiture failure. most importantly will destroy an LSD very quickly.
what you can do is put your spare on the rear and your rear tyre on the front to match your rolling diameter on the front, wont matter on the rear at all coz they are indipendant to the drive train.

I've also heard that if you have an accident while you have your spare tire on it's possible your insurance will not cover you- as all 4 tires are not the same size or something...

I doubt this would be true. The lawyers would have a field day with this if they ever denied a claim cause someone was in an accident after they got a flat tyre!
