I actually turned the boot inside out, pushed the adaptor ring through and used the standard plastic collar that came with the factory shift knob to secure the boot to the adaptor. I then wrapped tape around the shaft at the desired height to prevent the ring from falling down. Was very easy and I think it looks pretty good.
